Special Accommodations

If you are in need of special accommodations to assist with your learning experience, please start by reviewing our disabilities philosophy and mission statement. Before submitting a request for special learning accommodations, please read ADA accommodation procedures for Baker University.

Please review, print and complete the forms appropriate to your situation:

1. Voluntary Intake and Disability Disclosure form
2. Verification of Physical/Health-Related Disabilities form. Complete the first page of this form; pages 2 through 4 need to be completed by your health-care provider.
3. Academic Accommodations Request form. A separate form must be filed for each course in which you would like assistance.
4. If you would like us to notify your instructors of your disability and any special accommodations, please print and sign our
   Letters to Instructors form.
